Welcome to both our returning and new student-athletes! We hope that you are all having a safe and healthy summer. On this page, you will find the information that is required by the Athletic Training Staff for the 2010-2011 athletic year. Please note that the paperwork has changed from previous years.
This year Bryant Athletics will be going 50 percent paperless. Below, you will find a list of documents to be completed and mailed to the Athletic Training Room by July 23, 2010.
1. Annual Physical
completed by your Primary Care Doctor
2. Orthopedic Evaluation
completed by an Orthopedic Doctor (Freshman/Transfers ONLY)
3. Copy of the front and back
of your insurance card(s)
4. Proof of sickle cell
testing (Freshman/Transfers ONLY; required per the NCAA)
5. Documentation from
diagnosing/prescribing physician if diagnosed with ADD/ADHD
(required per the NCAA)

The remaining required documents can be found online via each student-athlete's Banner account by June 1, 2010. Student-athletes will receive an email from the Athletic Training Room when this paperwork becomes available and should complete it online to be submitted via Banner.
PLEASE NOTE THE FOLLOWING:
- Exceptions will NOT be made on physicals. Student-athletes MUST turn in the physical examination form found above prior to arrival on campus for preseason or the start of the 2010-11 academic year regardless of the date of his/her last physical.
- Paperwork due to the Athletic Training Room is completely separate from paperwork due to Bryant Health Services. All new students (freshmen and transfers) must download the necessary forms at www.bryant.edu/healthservices and return to Health Services by August 1, 2010, or else they will not be permitted on campus as per university regulations. Again, this paperwork is completely independent from the Bryant Athletic Training and both sets must be completed and returned prior to arrival on campus.
FOR FALL STUDENTS ARRIVING IN AUGUST FOR PRESEASON: If your paperwork is not submitted and received prior to your arrival for camp, you will not be cleared to participate. You will not be allowed to submit ANY paperwork the day you arrive for camp. If you do, you will not be cleared for the first day of participation. The deadline for your paperwork is July 23, 2010.
FOR WINTER/SPRING SPORT STUDENT-ATHLETES: If your paperwork is not submitted and approved prior to your arrival on campus, you will not be cleared for participation with your team. This includes: team lifting, team running, etc. You will not be cleared for ANY team activities, and no exceptions will be made. Your deadline for all paperwork is August 1, 2010.
ALL STUDENT ATHLETES

It is absolutely necessary that you get this done over the
summer.
FRESHMEN & TRANSFERS ONLY
An orthopedic evaluation needs to be completed prior to competition. This can either be done at home by your Orthopedic Doctor or by our team doctors upon arrival on campus for your season. Below you will find guidelines.
- If you will be getting the orthopedic evaluation at home you will print the form out under Orthopedic form. This form CANNOT be filled out by your general physician. It must be filled out by an orthopedic doctor and is a mandatory evaluation to have done.
- If you opt to get this done here at Bryant, our team doctors will be conducting these evaluations. There is a cost of $35 to get this done (cash or check). Checks can be written out to Bryant University. This will be collected when you get your evaluation done.
- It is required that during the orthopedic evaluations you wear attire conducive to this event. Male athletes will be asked to be in shorts and a t-shirt (the t-shirt will be asked to be removed in order to evaluate your back, shoulders and neck). Female athletes are required to be in shorts and a tank top. A female Athletic Trainer will be present during these evaluations for our female athletes.
