Bryant University Athletic Training Information Page
Welcome back to another year of Bryant athletics. We would like to review a few items and give you direction on what we need you to do with these forms considering we have changed them to make them easier for you.
Please submit all forms via mail to Bryant University by July 20th.
Please send all forms to:
Melanie Gran, Assistant Athletic Trainer Bryant University Athletic Training 1150 Douglas Pike Smithfield, RI 02917- EVERY student athlete must still get a physical from their General Physician. This has not changed. The form is still the same and you will find a separate link that will bring you to that form, allowing you to print it off and bring it to your doctor's appointment. It is absolutely necessary that you get this done over the summer so that we don't run into problems. NO PHYSICAL, NO PRESEASON. This has to be completed before you return to the field so it is highly suggested that you get this done at home.
- FRESHMAN & TRANSFERS, you have the option
of having an orthopedic evaluation done at home by your Orthopedic
Doctor or by our team doctors upon arrival on campus for your
season.
Downloadable Forms Form for All Athletes Orthopedic Form
- If you opt to get this done at home you will find a link providing the form that needs to be filled out by the orthopedic doctor. This form CANNOT be filled out by your general physician. It must be filled out by an orthopedic doctor and is a mandatory evaluation to have done when you are coming into Bryant Athletics.
- If you opt to get this done here at Bryant, our team doctors will be conducting these evaluations. There is a cost of $30.00 to get this done (cash or checks). Checks can be written out to Bryant University. This will be collected when you get your evaluation done.
- It is required that during the orthopedic evaluations you wear attire conducive to this event. Male athletes will be required to be in shorts and a t-shirt (the t-shirt will be asked to be removed in order to evaluate your back, shoulders, and neck). Female athletes are required to be in shorts and a tank top or sports bra. A female Athletic Trainer will be present during these evaluations. If there are any questions, problems, or concerns please contact the Athletic Training room.
Finally, when filling out the paper work, please follow these simple directions.
- The first link is for all athletes, returning, freshmen, and transfer. Please download, complete to Bryant University's Athletic Training Department
- You will keep pages 1-2.
- Please mail in the remaining and submit all required material (physicals, copies of insurance cards, etc.)
Hopefully this makes things easier for you all this year. And remember the sooner we get ALL of your paperwork the easier things will be. If we are missing any paperwork you will not be able to participate in your season until this is done! Have a great summer and we can't wait to see you all again!!!
If you have any questions, please do not hestiate to contact Patrick
O'Sullivan at 401-232-6073 or Melanie Gran at 401-232-6530.
